have Acrobat, please
go back to the Department of Workforce Education homepage and scroll
to the bottom of the screen. There will be a free download for
Acrobat.
Open the Enrollment Form document and
"Save As" to your hard drive. This file is in Excel
format. Complete the requested
information and e-mail it to the State office. Please do not
forget the Verification Forms in the spreadsheet.
